Start PS5 Safe Mode or recover a console stuck there
Use the two-beep startup, a wired controller, one-cable substitution, a 20-minute power cycle, and only the least destructive Safe Mode option.

Turn PS5 fully off, hold the power button again, and release it after the second beep about seven seconds later. Connect the controller by USB and press PS. If Safe Mode does not appear, prove HDMI, USB, and power cables one at a time, then power the console off, unplug it for 20 minutes, and retry. If PS5 repeatedly returns to Safe Mode, power-cycle it and choose Clear Cache and Rebuild Database > Clear System Software Cache. Back up first: Reset PS5 options delete user data.

Procedure
Do this in order.

Use the official two-beep startup
Hold the power button for three seconds to turn PS5 off. Once the indicator stops, press and hold power again, release after the second beep about seven seconds later, connect the controller by USB, and press PS.

Prove the three cable paths
If Safe Mode does not display or accept input, substitute the HDMI, USB, and power cable one at a time and retry entry after each change.

Perform the documented power cycle
Hold power until the console beeps and turns off, remove the power cable for 20 minutes, reconnect it, and attempt Safe Mode again.

Use the least destructive relevant option
When PS5 is stuck returning to Safe Mode, power-cycle first, then choose Clear Cache and Rebuild Database > Clear System Software Cache. Use Restart, Change Video Output, Repair Console Storage, Update System Software, or other options only for their documented purpose.

Stop conditions
Know when not to keep changing things.
